Family Care Network, Inc. (FCNI), a transitional housing provider in San Luis Obispo and Santa Barbara Counties has developed a transition-planning tool to help young people prepare for successful, self-sufficient lives. The Lifebook is an evidence-based workbook, developed by FCNI that provides a step-by-step guided journey through several areas of life with practical, achievable activities under each step. These life domains include: Planning & Organizing, Educational Achievement, Employment & Career, Community Supports, Personal Living, Finance & Savings, and Health & Safety.
The categories are progressive, and can start as a conversation with a young person and advance to independent activities. Each category is exhaustive in covering the important topics, and includes web-based resources and space for youth to write directly into the workbook. Jim Roberts, FCNI’s Executive Director reports that their youth really like using the Lifebook – “they have found it to be realistic and doable. The youth who complete it have not only gained important life skills, but have earned some real bragging rights.”
