The Wisconsin Department of Children and Families is seeking proposals for the Family Foundations Comprehensive Home Visiting Program (FFHV).
The program aims to improve outcomes for eligible families who reside in at-risk communities through the provision of voluntary, high-quality, evidence-based home visiting services and the identification and coordination of needed services.
The Home Visiting Model includes, but is not limited to:
- Recruiting and retaining clients;
- Providing initial and ongoing training, supervision, and professional development for staff;
- Establishing a management information system to track data related to fidelity and services; and
- Developing an integrated resource and referral network to support client needs.
