The Santa Clara County Mental Health Department (MHD) is seeking proposals to provide infant-family early childhood mental health services as part of the KidConnections Network (KCN) of Providers.
KCN services focus on families with children birth through age 5 in need of necessary mental health and developmental services in their home, community or other settings appropriate to meet their needs for safety, nurturing and support which will improve their opportunity for optimal health and development.
The target population is children birth through age 5 and their parents/caregivers, with emphasis on serving vulnerable children in public systems such as the Superior Court System, Child Welfare System (including Differential Response), Children of Youth in the Juvenile Probation System, Head Start, Early Start, Child Signature Program, Regional Centers, Women Infants and Children Program (WIC), Department of Alcohol and Drugs (DADS) including Dependency Wellness Court.
Services provided are throughout Santa Clara County and utilize best practice models within a home visitation framework. Services, whenever possible, are in a child’s home or community setting. This may include, but not limited to, intake and registration, home visitation services and therapeutic services.
