The Santa Clara County Behavioral Health Services Department (BHSD), Family and Children Division, is seeking proposals from qualified Community Based Organizations (CBOs) to provide the following program services:
1. Mental Health Services Act Transitional Aged Youth (TAY) Outpatient Services
A comprehensive program that will deliver Outreach, Brief Crisis, Outpatient Behavioral Health and Drop-in Center services to Transitional Aged Youth (TAY) (ages 16 to 25) consumers living in Santa Clara County. This program will serve underserved and unserved TAY consumers, with a special emphasis on young adults from foster care and juvenile justice systems.
2. Mental Health Services Act Transitional Aged Youth LGBT Outpatient Services
A comprehensive program that will deliver Community Outreach Education, Behavioral Health and drop-in center services to TAY consumers living in Santa Clara County. This program will serve underserved and unserved TAY consumers from the Lesbian, Gay, Bisexual and Transgender (LGBT) population.
3. Transitional Aged Youth (TAY) Full Service Partnership Services
An intensive comprehensive program for TAY ages 16-25 which combines components from the Wraparound Model, AB2034, and Transition to Independence (TIP) approach in a framework that addresses transitional needs of young adults.
4. Child Full Service Partnership (FSP Services
An intensive comprehensive program for youth ages 6-15 which reflects the core values of Wraparound Model. FSP Youth consumers will receive outreach and engagement strategies specific to their developmental needs and are able to access services beyond the scope of traditional outpatient care.
